A referral can be submitted by the
person requesting services, friend, family, other support person, or a
service provider.
Once a referral form is completed and
received at the office, the triage process begins.
For Forensic Intensive Case Management
(FICM) and Homeless Case Management (HCM) programs, the referral is
forwarded directly to the coordinator of those programs for review and
assignment of a case manager for assessment. The case manager will
connect with the person referred to determine eligibility for the
program. To check on the status of a referral for FICM or HCM, contact

For Intensive case management (ICM),
supportive case management (SCM), or the Assertive Community Treatment
(ACT) team, the referral is forwarded to the Single Point of Access (SPOA)
Specialist for triage. A member of the SPOA team will contact the person
referred to arrange for an initial intake appointment. During this
appointment, information regarding the referral process and program
descriptions will be reviewed.
Details regarding the person’s current
situation, struggles, needs, current support system, recovery goals, and
obstacles will be discussed. This information, along with the clinical
information and history provided on the referral form itself, will be used
to determine program eligibility and level of need. If the clinical
information is not known, records can be requested to provide that
information.
The number of referrals received
exceeds the number of enrollment openings. People are enrolled based on
level of need. Those individuals with the greatest need for additional
support will participate in a comprehensive assessment when a case manager
is available and be enrolled in the program that will best meet their
needs.
For people who are not eligible for
case management services or are looking at a significant wait time before
enrollment is available, suggestions for alternative services and linkages
to other agencies as appropriate can be made.
