Homeless Case Management
 
Homeless Case Management (HCM)

Provides comprehensive case management services for disabled and non-disabled men, women, and families who are homeless in the Syracuse and Onondaga County community.

The program will offer community based practical assistance, transportation to appointments, housing and employment assistance, and wrap around funds for start-up resources. Start-up resources may include security deposits, moving and storage expenses, transportation, furnishings, food, and other financial needs as identified in the person's individualized service plan.


Eligibility Criteria:

This program will serve any homeless individual, 18 and over, in Onondaga County regardless of gender, disability, marital or familial status, or social service/shelter agency affiliation. Individuals must be currently living on the streets or in a homeless shelter.

They may also be residing in transitional or supportive housing units for homeless persons originally from the streets, shelters, hospitals, or institutions. They may also be assisted with relocation if they have been to eviction court and been given notice to vacate the property if such eviction would leave them without resources, or supports, necessary to obtain housing.
