Growing human services agency seeking candidates for Care Managers. Work with individuals recovering from serious mental health or other chronic conditions.

 

Qualifications:

Master's degree preferred, bachelor's degree in a health or human services field or a CASAC required. Two years experience providing services to individuals with mental health disorders or substance abuse / dependence.

Health Home Care Manager - Specialty

Full-Time Care Management professional position working with individuals with chemical dependency and mental health issues in a dynamic, creative agency.

 

Qualifications:
