Health Home Care Manager

Seeking full-time professional to work as a Care Manager in an expanding, dynamic, and creative agency. Work with individuals to address their medical and behavioral health care needs.

 

Minimum Qualifications:

Master's Degree preferred OR Bachelor's Degree in a health or human services field, CASAC, RN or LPN and two years' experience providing services to persons in the medical or human services field required. Must have valid driver's license and own transportation. Multi-lingual a plus.

Health Home Care Manager - Dual Recovery

Seeking full-time Care Management professional to work with individuals with a chemical dependency, mental health and medical health care issues in a dynamic, creative agency.

 

Minimum Qualifications:

Master's Degree preferred OR Bachelor's Degree in a health or human services field, CASAC, RN or LPN and two years' experience providing services to individuals with substance abuse/dependence required. Must have valid driver's license and own transportation. Multi-lingual a plus.

Seeking full-time Social Worker / Clinician to work in a home and community based program for children with emotional disturbances. Provide intensive, short-term therapeutic intervention.

Master's Degree in Social Work, Child and Family Studies, or a related field with two or more year's experience, skills in child and family assessment, therapy and case management. Must have valid driver's license and own transportation. Multilingual a plus.

Progressive Not-For-Profit Agency with an expanding Mental Health Clinic is seeking an experienced Per Diem Psychiatrist to join our existing Psychiatric team. We will offer very competitive compensation. Our Clinicians are committed to providing excellent care in a professional setting. Anticipated need is for 8 hours per week which may increase as our Clinic continues to expand.

Full-Time Nurse to provide clinical support and care coordination service in the evidence-based ACT team model and serve as a Psychiatric Nurse.

 

Minimum Qualifications:

Registered Nurse (RN) with experience / knowledge treating individuals with a diagnosis of a serious mental illness. Valid driver's license and own transportation.

 

Preferred Qualifications:

Experience providing community based services and working as part of a muti-disciplinary team a plus. Multi-lingual a plus.

Seeking FT Human Service professional to promote Health Home's vision and mission and establish/maintain Health Home's name and image within communities/counties that we operate. Develop, manage, and execute a marketing and communication plan. Manage day to day marketing efforts. Must have strong organizational skills, demonstrated ability to communicate effectively both verbally and in writing, ability to work independently and be accountable for results, excellent public speaking and presentation skills, knowledge of the healthcare industry and Health Homes market preferred.

 

Minimum Qualifications:

Bachelor's degree in human services or related field required.

 

Preferred Qualifications:

Advanced degree in human services, MBA, or MPA preferred. Experience in sales, healthcare, marketing, public relations, business development, or social services preferred.
